Schengen Visa For Italy

Having read quite a lot of posts about Schengen visas, and got an appointment at the Italian Consulate, Birmingham, we want to make sure we have all necessary documents to get the visa. There are still a few things we are not clear about:

Does the marriage certificate translation need to be certified by the Home Office or Thai Embassy as offical? (Our translation was certified by a legal company in Bangkok)

Do we really need confirmed travel reservations and insurance (the travel agent can issue us with a quote only)

We ideally want a multiple entry visa for a visit to Italy in May and a visit to Spain later in the year, possibly October. Do we have to provide travel reservations for Spain as well?
